Just a reminder that ALL winter athletes must have the following paperwork on file in order to tryout.

1. Completed VHSL physical form (Dated after May 1st 2017)

2. Completed Acknowledgement form for Athletic Policies/Procedures (can be found outside athletic training room or under the files tab)

3. Completed Concussion History Form (can be found outside the athletic training room or under the files tab)

4. Current Impact baseline assessment
