VHSL Physical Form 2021
Beginning in the fall of 2021, this is the VHSL physical form that must be completed and signed by a Physician in order for your student-athlete to be able to participate in athletics.

The following form must be completed prior to each athletic season, informing the medical staff of any prior concussions an athlete may have sustained.
FCPS Policy Regulations and Protocol for Managing Concussions
This is the official FCPS concussion policy regulation and protocol. It explains what actions and steps will be taken in the event that your child suffers from a concussion during school activity. This form also lists the steps that must be completed for your child to return to physical activity after they have suffered from a concussion.

After reading the policy, regulation, and fact sheets, you will need to complete the concussion history form as well as the concussion policy acknowledgement form prior to your child competting in athletics here at KRHS.
